We'd like you to submit up to three (3) photos clearly demonstrating the areas of the face, teeth, or other areas of the head and neck that you want to improve. One of your photos must be a full-length photo of yourself.

Special Notes About Your Digital Photo Files:

All photo filenames must contain your full name and your phone number.  No spaces are allowed. 
Examples:
   JonRSmith5551212-1.jpg
   JonRSmith5551212-2.jpg,
   JonRSmith5551212-3.jpg.

The MAXIMUM photo file size accepted is 1 Mb (1000000 bytes).

To Upload Your Digital Photos:

(1) Click the "Browse" button below to open a browser to your computer.

(2)  In the browser window, locate and select your photo, then click "Open". You should see the location and filename of your photo appear in the space below.

(3) Click "Send Photo". Please wait patiently for your photo to upload ... photo uploads take time!

How To Tell If It Worked:

If your photo uploaded successfully, you will see a "Thank You" page. That page will automatically bring you back here so you can send additional photos. Remember, up to three photos per candidate will be accepted.

If you receive an error, most likely your photo file size was too large. Use your photo-editing software to resize your photo, then return to this page and try again. If you still receive errors, please send an email to the webmaster describing the problem.
